New consent form for giving access to someone to speak on your behalf.
Historically, we have received letters from patients requesting access rights for a family member, friend, or neighbour to make inquiries on their behalf. However, this system is vulnerable to misuse, and the practice must ensure that the request genuinely comes from the patient and that they fully understand the extent of the access they are granting.
To enhance Data Protection and Safeguarding, a more formal process has been introduced. A consent form has been developed for this purpose, please see the attached document.

After completing the consent form, the patient must visit reception in person with their ID for activation. We cannot accept ID delivered by someone else; the patient must be present when submitting the form.
